the Elan fax-back form when requesting a panel. The request must include;
• Clients name, address and date of birth • Clients telephone numbers - home/work/mobile • Accident date • Claim reference number • Brief nature of injuries • Type of specialist required Elan Medical will provide a panel of 3 suitable experts within reasonable distance of the client’s home. An Elan Case Reference Number must be quoted on all communications. |
